Each of the adoptive families we work with has different needs and concerns. Our unique approach to Wraparound Services allows us to create a customized plan of action for each family.
Through trauma-informed education, we create a therapeutic environment for your child to heal and thrive, while providing parents with the tools and resources they need to be supportive and present.
When you partner with us, the success of your family isn't gauged by the number of providers managing your child's needs at home, but rather by the overall interaction and cohesion within your family unit.
This approach enables you to gradually reduce reliance on extensive provider support, fostering more quality family time and enhancing your collective well-being.
Do I Qualify?
You May Qualify For Services If:
Your child is under 18 (or up to 21 with AAP + prior Wraparound)
Your child was adopted from California
You currently live in California
You receive or qualify for Adoption Assistance Program (AAP)
Your family is experiencing significant challenges and needs intensive support
Your child has emotional, behavioral, educational, or relational challenges that require extra support.
Your child is at risk of placement outside the home or is returning from a higher-level setting (such as an STRTP or residential program).
Your child may meet medical necessity if they are at risk of entering residential treatment, transitioning home from an STRTP or out-of-state facility, or transferring from another Wraparound program.
You have concerns about family stability or the possibility of a placement disruption.
Our High Fidelity Wraparound model uses four distinct phases that guide and support the Wraparound process.
These phases include:
Phase 1
Engagement (Approximately 1 month)
Build trust with the youth and family
Orient family to the Wraparound process
Identify immediate needs and safety considerations
Begin exploring strengths, needs, and a family vision
Prepare the Child & Family Team (CFT) for the planning phase
Phase 2
Planning (Typically 1–2 months)
Facilitate first Child & Family Team meetings
Develop the individualized Plan of Care
Create strategies based on strengths and underlying needs
Assign team roles and responsibilities
Establish a crisis/safety plan
Phase 3
Implementation (Duration varies based on family needs)
Deliver supports and interventions outlined in the Plan of Care
Monitor progress toward outcomes
Adjust strategies and action steps as needs change
Strengthen natural supports and family voice/choice
Continue regular CFT meetings
Phase 4
Transition (Typically begins 3-6 months before exit)
Prepare family and natural supports for life after formal Wraparound
Celebrate progress and successes
Identify ongoing community/natural supports
Create a post-wraparound transition plan
Establish follow-up and sustainability steps
